Patient Resources: Forms and Care Cards

We have gathered the patient information forms you can fill in before your first examination and the care cards that will help you during treatment on a single page. All documents can be downloaded and printed as PDFs.

  • Clear aligner and retainer care cards

  • Downloadable and printable PDFs

The documents on this page are designed to make the examination and treatment process easier for you. You can fill in the forms at home in your own time and bring them with you to the examination, and you can print the care cards and keep them next to the bathroom mirror or inside your aligner case. The documents are for general information; your own instructions are always the ones your orthodontist gives you.

Forms to fill in before the examination

At the first orthodontic examination you will be asked about your general health, the medicines you take and any previous dental treatment. Thinking about this information and writing it down in advance saves time for both you and your orthodontist and helps make sure no important detail is forgotten.

Filling in the form is not compulsory; if you prefer, we can go through this information together at the examination. Please hand in the completed form only in person at the clinic; we recommend that you do not send your health information by email or social media. You can find out how personal data is processed on the privacy notice (KVKK) page.

During treatment: care cards

There are also interactive on-screen versions of these cards: the clear aligner daily care card, retainer cleaning and care and the lost or broken aligner guides. To see the dates for switching to your next aligner, you can use the aligner change calendar tool.

What to bring to the examination

  • The patient information form, if you have filled it in
  • An identity document; for child patients, the parent's ID
  • The names of the medicines you take regularly
  • Any dental X-rays taken within the last year or previous orthodontic records
  • Any aligner, retainer or appliance you have used before
  • Your policy details, if you have private health insurance

To prepare the questions you would like to ask your orthodontist, take a look at the examination question list page. If you are coming from another city or from abroad, you can send photos for an online preliminary assessment before the examination.

Frequently asked questions

Do I have to fill in the forms?

No. The forms are only there to make preparation easier. The necessary information can also be taken together with you at the examination.

How should I hand in the form?

Simply bring the completed form to the clinic on the day of your examination. As it contains health information, we recommend that you do not send it by email or a messaging app.

Are the care cards the same for everyone?

The cards summarise the general rules. If your orthodontist has given you different instructions on things like wear time, how often to change aligners or how to use your retainer, always follow those instructions.

Which languages are the documents available in?

The documents are available in Turkish, English, German and Russian. The clinic team can communicate in these four languages.

For questions, you can call 0312 911 60 36 or use the form on the contact page.

This page is for general information only; diagnosis and the treatment plan are determined by an examination and your orthodontist's assessment.
