Short answer: IPR (interproximal reduction, also called "stripping" or enamel reduction) creates a small amount of space by removing enamel from the sides of the teeth at the level of tenths of a millimetre. It is done in limited amounts, on teeth chosen in the treatment plan, to provide the space needed to correct crowding and to make tooth shapes more harmonious.
Why is IPR done?
- Creating space: Providing room for teeth to line up in mild to moderate crowding without needing extractions
- Reshaping teeth: Reducing dark triangular gaps at the gumline between triangular-shaped teeth (black triangles)
- Balancing the upper and lower jaws: Evening out differences in tooth width between the two arches
How much enamel is removed?
The amount to be removed and the teeth involved are set in advance in the digital treatment plan. Only very small amounts are usually involved at each contact point, and the procedure is planned within the safe limits of enamel thickness. It can also be split across different stages of treatment rather than done all at once.
How is it done?
- The planned teeth are identified and the contact points between them are checked.
- Enamel is removed from the sides of the teeth using thin abrasive strips or special discs.
- The space created is checked with a measuring gauge.
- The surfaces are polished; protective fluoride is applied if needed.
The procedure usually doesn't require local anaesthetic; you may feel brief vibration and a sense of pressure.
Does it damage your teeth?
There are studies showing that when IPR is done for the right reasons and in the planned amount, it does not increase the risk of tooth decay or gum disease. Even so, some sensitivity to hot and cold may be felt for a few days afterwards. Good oral hygiene and regular check-ups are important.
Who may not be suitable for it?
- Teeth with thin or worn enamel, or widespread decay
- People with untreated gum disease
- People whose oral hygiene is inadequate
- Severe crowding where the space needed cannot be gained with IPR
How the space is gained differs from person to person. For types of crowding and treatment options, see our page on treatment for crowded teeth.
Quick answers
Is IPR done in every clear aligner treatment?
No. It is only used when the treatment plan calls for extra space or reshaping.
Will there be gaps between my teeth after IPR?
The small spaces created close up as the teeth move during treatment.
What is the difference between IPR and tooth extraction?
IPR creates space measured in fractions of a millimetre; when more space is needed, other options are considered.
How do you find out what suits you?
The information here is general; the right plan for you is decided through an oral examination, a digital scan and a clinical assessment. At our clinic in Oran, Çankaya, Ankara, we go through these topics and your questions during your first orthodontic examination. If you would like to see the treatment steps in advance, take a look at our clear aligner treatment process page.
