Short answer: Tongue thrust is when the tongue pushes between or behind the front teeth during swallowing or speaking. If it continues for a long time, it can contribute to an open bite. A tongue crib appliance (tongue guard) stops the tongue from reaching the teeth and helps to break the habit; it is often used together with tongue exercises.
Signs of tongue thrust
- A gap remaining between the front teeth when the teeth are closed
- The tongue being visible between the teeth when swallowing
- A lisp on “s” and “z” sounds
- Lips that close with tension and strain

How does a tongue crib work?
At the front of a fixed or removable appliance attached to the upper molars there are thin wire spurs or a barrier that stop the tongue pushing against the teeth. Over time the tongue gets used to a new resting and swallowing position behind this barrier.
Why are exercises important?
An appliance on its own does not always change the habit permanently. Myofunctional exercises that teach the correct tongue position can be carried out, together with a speech and language therapist if needed. Causes such as mouth breathing, enlarged tonsils and allergies should also be assessed.
How long is it worn?
- Usually a few months; improvement of the habit is monitored at check-ups
- It may need to be worn for a while after the habit has been broken
- Speaking and eating may be harder in the first days
