Short answer: Headgear is an extraoral orthodontic appliance that rests on a neck or head strap and controls the upper molars or the growth of the upper jaw. It is mostly used in growing children and teenagers and is worn in the evening and at night as instructed by the orthodontist.
When is it used?
- To slow excessive forward growth of the upper jaw
- To hold the upper molars back or move them backwards
- To create space for the front teeth to line up
For general information on protruding teeth, see our page on treatment of protruding teeth.
Types
- Cervical (neck) headgear: The force comes from a neck strap
- High-pull headgear: The force comes from the top of the head; preferred when vertical control is needed
- Combination headgear: Takes support from both directions
How is it put on and removed?
- Insert the inner arms of the face bow into the tubes on the molars.
- Put on the neck or head strap and attach the safety release.
- To remove it, first detach the strap, then carefully pull the face bow forward.
Running, playing games or contact activities should not be done while wearing headgear. This safety rule is explained to both the child and the parent.
Why is it used less often today?
Mini-screws, fixed functional appliances and clear aligners can replace headgear in some situations. Even so, headgear is still considered an effective option for certain growth problems. For mini-screws, read our article on the orthodontic mini-screw.
