The short answer: aligners are a treatment, not a product
Invisalign is not bought like a boxed product. What moves your teeth is the plan behind the aligner as much as the aligner itself. And the plan is built and approved by a doctor who examines you. That is why Invisalign is provided only through an Invisalign provider doctor.
This information is general; the treatment that suits you is decided after an examination.
How does the doctor route work, step by step?
- Examination. The doctor looks in your mouth. Cavities, gum condition, the jaw joint and missing teeth are assessed.
- X-rays and records. A panoramic X-ray is requested for most people. If needed, additional films and photographs are taken.
- Digital scan. Your mouth is scanned with iTero. An impression tray may not be needed.
- ClinCheck plan. Where the teeth will go, step by step, is shown in 3D on the screen. The doctor edits the plan and you see it too.
- Approval and manufacturing. After the plan is approved, the aligners are manufactured.
- Check-ups. The aligners are delivered. The doctor monitors progress at intervals.

Why are the examination and X-ray necessary?
Moving teeth also means moving roots and bone. The condition of the roots, the bone support and the structure of the jaw cannot be understood from outside the mouth. An X-ray shows them.
If there is an active cavity or gum disease, that is treated first. Otherwise tooth movement can make the problem worse. For some people aligners alone are not enough. The doctor says so from the start and suggests another route.
Why does ClinCheck approval matter?
ClinCheck lets you see the goal of the treatment. But its real value is that the doctor can change the plan. The doctor decides whether a tooth will move more or less, where the attachments will go and whether space will be created.
Manufacturing does not begin until the plan is complete. This reduces mistakes that are hard to fix later. The what is Invisalign page, which describes the steps of the method, is also useful.
Why are there staged check-ups?
Aligners are given in sets lasting a few weeks. With each set, it is checked whether the tooth has moved as expected. The plan is updated if needed. Check-ups are usually held at intervals of a few weeks; the doctor sets the interval.
If an aligner does not seat, an attachment has fallen off or a tooth is going differently than expected, only an in-person examination catches it. When treatment ends, the retention plan is also made with the doctor.
The difference from aligners bought online or by mail
| Topic | Invisalign through a doctor | Online or mail-order aligner systems |
|---|---|---|
| Examination | In person, at the clinic | Mostly through photos or an impression kit |
| X-ray | Requested if needed and interpreted by the doctor | Mostly none, or depends on the person bringing one |
| Plan | ClinCheck, approved by the doctor | The plan can proceed without a doctor seeing you |
| Attachments and, if needed, space creation | Done by the doctor | May not be possible in every system |
| Check-ups | At intervals, according to how the plan progresses | Remote, or none |
| When a problem comes up | You come to the clinic | It may be unclear whom to contact |
The table is a generalization. Not every remote system is the same. But the shared risk is this: if there is a root, bone or gum problem in your mouth, teeth may be moved without anyone noticing.
A remote pre-assessment is not forbidden
A remote pre-assessment directed by a doctor can be useful. You send your photos and the doctor tells you whether you appear suitable. But this is a preliminary consultation, not the treatment itself. The clinic's online pre-assessment is done for this purpose; afterward an in-person examination is needed.

Decisions about attachments and space creation
Moving some teeth requires small tooth-colored attachments. If the teeth are tightly spaced, the doctor can create a very thin gap. How much of this to do and where depends on your examination findings. In a system without a doctor, no one makes this decision. Either it is not applied at all or the aligner falls short. As a result the aligner does not seat, the tooth does not go where it should, and you cannot understand why.
A real-life scenario
In the third month the aligner begins to feel tight and one tooth lags behind. If you are working with a doctor, you come to the clinic, your mouth is scanned, the plan is updated and a new set is produced if needed. In a remote system, you are often advised to move on to the next aligner. The aligner seats even less and the problem grows.
When you come to your appointment
For the first visit, bring your old X-rays if you have them, a list of the medications you use and information about any orthodontic treatment done before. Write down your questions. Finding out whether your mouth is suitable before choosing an aligner system saves both time and effort.
Three questions you can ask when choosing a doctor
- Are you an Invisalign provider, and will you scan my mouth digitally?
- Will you show me the ClinCheck plan and review it with me?
- How are check-ups planned, and what do we do if an aligner is lost?
